Follow these step-by-step instructions to access your 401(k) fee disclosure online.


  1. Log in to Fidelity
  2. Click on the 'Plan information' tab
  3. You'll see a long list of options, one of which should be the 'fees in the plan'.

  2. From the orange menu bar at the top of the screen, select 'Plan info'
  3. Click 'Sponsor Disclosure', followed by 'Participant/ Plan Sponsor Disclosures'


  1. Log in to Ascensus with a User ID and Master PIN
  2. Click 'Plan Disclosure'
  3. Click on the latest Participant Disclosure in the box at the top right.

  3. Click on 'Required Disclosure'
  4. Click on 'Participant Fee Disclosure'
